- Computerized Tomography (CAT Scan)
- Magnetic Resonance Imaging (MRI)
- Human Organ/Tissue Transplants
- PET Scan
- Cardiac Nuclear Scan
- Chemotherapy
- Hemodialysis
- High-Cost Medications/Specialty Medications
